The TarGeT-A protocol is part of a larger trial aimed at assessing the efficacy of ribociclib and everolimus in patients with newly diagnosed High Grade Glioma (HGG) or DIPG as well as the efficacy of ribociclib and Temozolomide in patients with DHG. This treatment focuses on tumours that have specific changes in the cell cycle or growth pathways, as well as certain tumours with a genetic change called H3G34-mutant. The study also explores markers of how the disease responds to treatment, looks for new targets for immunotherapy, and uses advanced imaging to track the tumour. Throughout the study, patient reported outcomes are monitored to ensure quality of life is maintained during these personalised treatments.
TarGeT-A is a molecularly guided, Phase II arm of the CONNECT TarGeT adaptive umbrella trial. It evaluates maintenance therapies in paediatric and young adult patients with newly diagnosed HGG, DIPG, or localised diffuse hemispheric glioma (DHG) following radiotherapy. Patients are stratified by molecular alterations: those with cell cycle or PI3K/mTOR pathway dysregulation receive ribociclib and everolimus, while H3G34-mutant DHG patients receive ribociclib and temozolomide. Primary objectives include assessing event free and overall survival. Secondary endpoints involve evaluating biomarkers of response and resistance and validating alternative imaging biomarkers.
